2012 UWTA Regional Tournament

Important Liability Waiver Information:
Please read this carefully before signing up for the tournament.
I hereby submit this registration with the clear understanding that a liability waiver is needed to participate in the 2012 UWTA Regional Tournament. I certify that the registration information submitted is true and correct and hereby release, discharge and waive any and all responsibility of Robinson’sTaekwondo Centers, United World Taekwondo Association, the Tournament Organizing Committee, tournament directors, referees, instructors, coaches, medical staff and other competitors from liability for any injury, including death, and for damage to or loss of property which may be suffered by myself arising out of, or in any way resulting from or attributing in whole or in part to my traveling to, training for, being coached in, using any sports equipment, or participating in the 2012 UWTA Regional Tournament at Mather Gym in Rancho Cordova.
I have read and understood the above waiver. I understand without my acceptance that participation in the event will not be allowed. I agree that my name and contact information connected with the registration of the participant is correct as the Parent/Guardian of the Participant or as the Participant, and that I personally completed this form and registration. I also understand that by selecting my acceptance in the drop down box below that I am acknowledging the release for purposes of this Liability Waiver.
